Graduate of Phelps, friend of the school district, employee

Minimum age 35

Significant and recognizable contribution to society, profession or other outstanding achievement

Integrity/Ethical

Nominations

Completed nomination form due by January 1 in the school district office (mail or fax).

Can be nomiated posthumously.

Nomination form and information on school website, school district office and Phelps Chamber office.

Attach supporting documents (Certifications/newspaper articles/honors, etc.) to nomination form.

Nomination Application/Letter-- stating why the nominee should be selected, detailing their contributions personally/professionaly to the school district or society as a whole.
